Referring to a Clinic or Service

Step 1
Referring professional identifies that patient requires consultation at one of the specialty clinic/service areas at SickKids proceeds to the eCHN login page to complete the web-based Ambulatory Referral form.

Step 2
Referring professional or delegate identifies the clinic or service needed.

If the clinic or service is not listed, referring professional or delegate contacts clinic/service required for information and instructions on how to submit the referral.

If the contact information for the clinic/service is unknown, the referring professional will telephone the main switchboard at 416-813-1500 and ask to be connected to the clinic/service needed OR
Referring professional or delegate selects a “clinic” from the drop down clinic list on the Web page. Clinics/Services are sorted alphabetically.

Step 3
Referring professional or delegate selects a “problem” (e.g. a diagnosis, problem, test or procedure) from the drop down problem list.

Referring professional or delegate reviews the guideline for the problem for, patient criteria, pre visit requirements and appropriateness. Review of the ‘educational link ‘ (if available).

Note: The delegate can print this page for the referring professional to review before entry of information.

Step 4
Referring professional or delegate completes the information on the guideline page, patient’s legal name, supporting information and reason for referral.

Referring professional or delegate enters the information for the referring professional. If the referring professional is a physician and have entered their billing number, their demographic information will be held for subsequent referrals.

Referring professional or delegate then completes the online referral form,, prints a copy for his/her record and then submits referral form electronically. Once your referral is submitted you will be given a referral ID. Please note the referral ID number on all supporting documents being sent for that specific referral.

Note: If you do not see a referral ID or if the referral ID is recorded as a series of zeros, the referral has not been sent. Please try to resend, ensuring there are no special characters such as *\ in the reason for referral field.

Step 5
If a test or procedure is selected from the “problem list” a requisition will be required.

The referring professional must first submit the completed referral form, then print off and complete the requisition required for that test or procedure.

The referring professional will then fax the completed requisition to; eCHN (if original referral submitted through eCHN) or directly to the appropriate clinic/service area (if original referral faxed or sent via ARMs application).

Step 6
Referring professional prints a cover page with referral ID and faxes or mail any supporting documents for that referral or requisition to the appropriate clinic/service area selected.

The referring professional will then fax the supporting documents to; eCHN (if original referral submitted through eCHN) or directly to the appropriate clinic/service area (if original referral faxed or sent via ARMs application).

Please refer to the Directory of clinics or contact the specific clinic/service area through switchboard for their ARMs fax number, if unknown.

Step 7
Referring professional will be notified automatically via fax that the submitted referral has been received and is being reviewed by a SickKids professional.

For eCHN e-Referral submissions; the referring professional will be notified via email of a referral status change and instructed to log on to their eCHN account for details of the status change.

Step 8
A SickKids professional receives and reviews/triages the referral. The SickKids professional may request more information from referring professional if insufficient information is received to triage the referral appropriately.

If the referral is inappropriate for the clinic specified, the referral may be redirected to another clinic inside SickKids or to an external SickKids partner or an alternate plan may be suggested to the referring professional.

The referring professional will receive an automated message informing him/her of the status of referral (e.g. under-review, redirected/forwarded to another professional, alternate plan suggested).

Step 9
A SickKids professional books the patient, and the referring professional receives an automated fax message informing him/her of the appointment status of the patient.

For eCHN e-Referral submissions; referring professional will be notified via email of a referral status change and instructed to log on to their eCHN account for details of the status change..

Step 10
The SickKids clinic/service area informs the patient and/ or family of the appointment schedule via telephone or mail, with appropriate accompanying information/instructions for appointment.

If mailing a referral:

  • To ensure referral reaches the appropriate clinic when sending the form by mail, include the specific clinic name in the mailing address. See directory of clinic.
  • When referral is requested for patient to be seen within a short time frame, provide the family with the “coming for clinic appointments” information in order to prepare them for their visit.
