Occupational health requirements

Compliance with Hospital Staff Immunization and Surveillance Policy 

For the health and safety of patients and staff, and in compliance with the Communicable Disease Surveillance Protocols for Ontario Hospitals, and the policies of The Hospital for Sick Children, your employment is conditional upon you providing Occupational Health & Safety Services, in accordance with the Deadline For Fulfillment of Condition provision set out below, with satisfactory documentation confirming the following:

a) Tuberculosis (TB) 2-step skin test showing completed results:

You must provide a documented 2-step TB skin test showing completed results. Please refer to the enclosed Staff Immunization & Surveillance Policy information sheet regarding TB skin testing process. Please note that it can take up to two weeks to complete a 2-step TB skin test; and

b) Immunization status:

You must provide satisfactory documentation of immunity to rubella, measles, mumps and chickenpox as well as your Hepatitis B immune status. Please refer to the enclosed Staff Immunization & Surveillance Policy information sheet regarding acceptable documentation. The attached Immunization Record (Appendix A) should be completed by either a physician or, if appropriate, the Occupational Health Nurse at your previous employer. You are required to bring this completed documentation to your Occupational Health appointment which will be scheduled during the first week of your employment.

A completed Appendix A, the Health review form should be brought to your Occupational Health appointment. This information is kept confidential within Occupational Health & Safety Services. Any costs associated with the completion of the form and/or collecting the information will be your sole responsibility.

Deadline for Fulfillment of Condition: If you cannot provide proof of your completed 2-step TB skin test results and proof of Immunization (together "Results") to your Occupational Health appointment, you will be conditionally permitted to start your employment, conditional upon providing the Results within fourteen (14) days of your expected start date ("First Grace Period"), during which time you will be put on a monitoring protocol. If you do not provide the Results within the First Grace Period, you will be placed on an unpaid leave of absence for a period of up to an additional fourteen (14) days, pending receipt of the Results ("Second Grace Period"). If you do not provide the Results by the end of the Second Grace Period, you will not have met a condition of your employment, and this offer of employment will be rescinded.

All personal health information will be kept confidential. Any costs associated with collecting the information pertaining to your TB and immunization status will be your sole responsibility..
