How to make a referral
Referrals to Dermatology clinic are accepted from Paediatricians, Dermatologists and Family Practitioners. Please complete the standard SickKids referral form and forward it to us by mail or fax. We have a prioritized referral system; each completed referral is triaged within five business days of receipt. Families and Physicians will be notified directly of the appointment, by phone and mail.
Prior to your clinic appointment you will be sent a Welcome to Our Clinic pamphlet which includes clinic directions and will allow you to identify any translation needs.
