Transition to Teen

Goals of this Program:

  • To provide an information session for parents and teens related to changes that occur during adolescence and the effect of these changes on coping with diabetes.
  • To provide an opportunity for parents and teens to problem-solve around conflicts and challenges related to having diabetes.
  • To reinforce the "Concept of Balance" and how to achieve it through the young teen years.

This program is designed for teens with diabetes age 12-14 years and their parents.

Diabetes nurse educators, physicians, and social workers will be on hand to help facilitate the discussions.

Program Objectives:

By the end of the session, teens will be able to:

  • State the effect of insulin, food, and activity on blood sugar levels.
  • Identify other factors that affect blood sugar.
  • Explain how blood sugar balance can be evaluated.
  • State personal goals.
  • Discuss challenges in diabetes care related to transitions of adolescence.
