Graduation

More and more, we are trying to use the terminology of graduation rather than transfer with young people. Programs in other hospitals often incorporate some way to mark the graduation of a teen, whether this is by presenting them with a certificate, making sure that everyone has a chance to say goodbye, or even a graduation ceremony held during a transition clinic or transition day. Hospital staff who have had an affiliation with the patient can be invited to attend to celebrate this next milestone in the teen’s life.

We have links to two graduation certificates. One is a SickKids graduation certificate, the other is a more generic one You can print it in colour and then fill in the patient's name and the name of your program. Get two people who have known the teen to sign it (doctor, nurse, social worker...) and present it to them.

It can be very helpful to give graduating patients a transition booklet, called Getting Ready for Adult Care, that details where they are going and who they will be seeing, as well as some general pointers about the adult system. This can be given to them on their last visit.  Copies of the booklet are available from the Good 2 Go Team.
