Timelines
These large posters are designed to highlight how a child with a chronic condition can move along a developmentally appropriate continuum towards increased self-management.
Information on the posters outlines age-appropriate tasks for growing up and will prompt families to think about their children’s futures and will provide ideas for how to assist their children in taking a more active role in their own health-care management.
The posters will be modified to meet the needs of children with varying conditions and will be posted in clinic waiting areas. They will be adapted from the Bloorview Kids Rehab Growing Up Ready posters, and can be adapted for the needs of families in various clinics and programs.
Transition materials
Brochures and web resources geared to various stages of transition complement the posters, with information parents and teens can take home. Online checklists have been developed for the PKU program at SickKids.
