Psychoeducational transition groups

Psycho-educational transition groups for parents

Parents of transitioning youth with a variety of chronic health issues will be invited to attend a bi-monthly group. Topics for discussion will include; developmental tasks of adolescents, encouraging self management skills, educational/vocational/financial planning, advocacy, and finding support through other parents and community organizations. These groups may be run at the same time as psycho-educational groups for youth.

Psycho-educational transition groups for teens

Any adolescent from any clinic will be able to attend groups for teens with chronic health conditions. We are open to having these groups in different formats--bimonthly meetings, intensive weekends, monthly half-days, etc. A variety of topics will be discussed including self advocacy, independent behaviors, sexual health, social support, accessing healthcare, educational/vocational/financial planning and healthy lifestyle issues. Groups will be facilitated by Good 2 Go team members.
