Training seminars will be provided for health-care professionals at one, or preferably both, the paediatric and the adult health-care centers. Topics offered will include; facilitating effective transition, encouraging self management, differences between the paediatric and adult health-care facilities, effective and kind termination practices, and quality care during transitions.
