Timelines
Transition preparation should start at the time of diagnosis. Parents often ask for guidance about how to raise their children to be able to take care of themselves and have normal lives.
Our Ready, Set, Good 2 Go Timelines (adapted from similar tools created by transition specialists at Seattle Children's and Holland Bloorview Kids Rehab) offer advice to parents of younger children and to teens about how they can develop their independence socially, in the family, at school and in medical issues in a colorful format. These are currently available for six disease areas as both large posters for clinics and a smaller poster (17 x 11) for families. These smaller versions also have helpful resource lists on the back.
