Coverage
Orthoses covered by the Assistive Devices Program (ADP)
Certain orthoses are paid in part by the Ontario Ministry of Health and Long-Term Care through the Assistive Devices Program (ADP). ADP pays for 75% of orthoses covered by the program.
Orthotic devices covered through ADP
- Specified custom-made braces/splints
- Selected pediatric orthoses
How to get ADP coverage
1. Your orthotist will give you an ADP Equipment/Supply Authorization (ESA) form if you are eligible to receive ADP coverage.
2. Bring the ESA form to your referring physician who will fill in the following: date, diagnosis, what they are prescribing and signature. Any medical physician can prescribe ADP funded orthoses.
3. Bring the ESA form back to The Centre for Orthotics.
(4. Your physician may provide you with an ESA form before you come to The Centre for Orthotics, but it is not a problem if he or she does not.)
For further information on the ADP program and answers to frequently asked questions, visit The Ministry of Health and Long-Term Care.
Extended health insurance
Some extended health insurance plans will pay for the remaining 25% of the cost of your orthosis. Simply pay the remaining cost at The Centre for Orthotics at the casting appointment and we will issue you a receipt that you can submit to your insurance company.
Charitable Organizations
Some charitable organizations may assist in paying for some or all of the remaining 25% cost of your orthosis if you do not have extended health insurance. You may or may not qualify for this assistance. It is the responsibility of the family to apply for assistance with charitable organizations.
Social Assistance
If you are receiving social assistance benefits under Ontario Works (OW), Ontario Disability Support Program (ODSP) or Assistance to Children with Severe Disabilities (ACSD), ADP may cover 100% of the cost. If your child is a patient at SickKids, speak to your clinic nurse or social worker for more information. For patients not affiliated with SickKids, please speak to your health care professional. Proof is required if you are receiving social assistance benefits.
