Appointments and referrals
Patients must have a prescription or a referral with a diagnosis from a physician before making an appointment with The Centre for Orthotics. Any physician can refer you to our department. If you do not have a prescription or referral, please visit your physician before contacting The Centre for Orthotics.
Your initial visit with a Certified Orthotist is free of charge; however, if the patient/family agrees to a treatment plan and is casted for an orthosis, a deposit is required during the visit. Payment is also required for off-the-shelf orthoses.

It is preferable if your physician fills out a referral using the SickKids Ambulatory Clinic Referral Form and faxes the form and your prescription to 416-813-5957. Once this is complete, you will receive a call from The Centre for Orthotics to book an appointment.
