SickKids Pilot Supported by AstraZeneca Award for Innovation

SickKids Nurse Practitioners Krista Keilty and Cathy Daniels received the Nurse Practitioners Association of Ontario-AstraZeneca Award for Innovation in Chronic Disease Management, a $5,000 grant supporting a pilot to bring community care to vulnerable children living with asthma, a priority for local Community Health Centres.

Working alongside TC LHIN Community Health Centre providers in a three-month pilot that launched this fall, paediatric nurse practitioners will implement clinical guidelines for the care of children with asthma.  Weekly clinics will offer patients consultations by NPs, as well as case reviews and training for health providers. The program is grounded in Keilty’s successful six-week experience at the Davenport-Perth CHC earlier this year.

This unique collaboration seeks to improve access and quality of care as well as chronic disease self-management, building paediatrics capacity by CHC health providers.

Ultimately these measures aim to improve system efficiency by reducing the number of urgent care and ED visits, and sub-specialty consultations.
