Transition treatment clinic
Good 2 Go has a weekly clinic for SickKids patients (12-18) who are struggling with transition issues--adherence, acceptance of their condition, becoming more autonomous. The clinic does not arrange adult care--this must be done by the home program.
Patient referrals for adolescents (ages 12-18)
A combination of medical and psychological issues that complicate their transfer to the adult health care system
complex and/or multiple medical issues that are inhibiting the individual's psychosocial development or transition through appropriate developmental stages
To refer a patient:
- To refer through the Ambulatory Referral Management System - (ARMs)- log in and choose "Good 2 Go" 7704
- If you aren't in ARMs, fill out a SickKids referral form
- Fax it to Sue Duncan at 7704.
- This will be reviewed within one week. We might give you a call to clarify issues
If the referral is not appropriate for Good 2 Go, it might be for the SickKids Interprofessional Team for Youth (SITY) (also in Teen Clinic). We will pass the referral on to them.
Patient will be seen in Teen Clinic on a Wednesday afternoon. We might be able to see them at another time if they have to travel to get here for other appointments.
