Transition Tools and Resources
Good 2 Go is based on the Shared Management Model, developed in Seattle, Washington. The model describes the development of an early therapeutic alliance between the children/youth, families and health-care providers as being essential to allow young people with chronic conditions to develop into independent, healthy, functioning adults (Kieckhefer & Trahms, 2000).
There is a strong emphasis on a gradual shift in responsibility where the leadership for care is shifted from the health professional to the parent then ultimately to the youth.
As the child ages, they actively participate in their health care in an age-appropriate manner. For example, a three-year-old child who takes insulin can walk to the refrigerator to get their insulin then give it to the parent who will administer it.
The roles of children, parents and the health-care team change over time and may move back and forth as the child's medical condition changes. The health-care team and parent eventually move into consultant roles and the teen, with new skills in self-management, eventually becomes the supervisor of their health.

On the left you will see our "menu" of items for transition. Some can be implemented by a group within SickKids with little help from Good 2 Go. Others will involve our input, while a few will be offered as a direct service by Good 2 Go.
