Our referral system: Please contact the SCAN Program to discuss all referrals with a clinician. This will allow appropriate triage, following which you will need to complete the standard SickKids referral form, and forward it to us by mail or fax depending on the urgency. An appointment for clinic visit will be booked in accordance to the nature of the patient's condition. Due to the nature of the services provided by SCAN program, self referrals will be accepted.
Please Note: Please provide physical examination findings, testing results, including swab results, x-rays, photographs etc.
