Keeping track of your child's medical history

SickKids offers tools to help you keep track of your child’s medical history. You are encouraged to complete one of these tools, keep it with you and show it to your child’s health-care providers, when needed.

MyChart

MyChart is a secure, online patient portal that provides patients and authorized family members and caregivers access to parts of their SickKids’ electronic health record, anywhere, at any time. MyChart does not display the complete historical SickKids patient health record. By default, patients will be able to see their lab results, send a secure message to their care team, update their medications, allergies and personal information, and more. Learn more.

MyHealth Passport

Visit www.sickkids.ca/myhealthpassport. After you fill out the online form, it will generate a wallet-sized card and a file you can download to your cell phone or other portable device. It lists your child’s medical conditions, past procedures/treatments, allergies and other health issues. It will also list your child’s medications.

Medication History Worksheet

It is important for your child’s health-care providers to know what medications your child is taking at home, so that they can make sure your child is getting the right medications while in the hospital.  After you fill out the Medication History Worksheet, show it to your doctor, nurse or pharmacist.
