Keeping track of your child's medical history
SickKids offers tools to help you keep track of your child’s medical history. You are encouraged to complete one of these tools, keep it with you and show it to your child’s health-care providers, when needed.
Visit www.sickkids.ca/myhealthpassport. After you fill out the online form, it will generate a wallet-sized card and a file you can download to your cell phone or other portable device. It lists your child’s medical conditions, past procedures/treatments, allergies and other health issues. It will also list your child’s medications.
This personal journal allows you to record important information about your child’s health. A special section also allows you and your child to express your personal thoughts and feelings and any questions you may have. To download the Family Journal, click here.
Medication History Worksheet
It is important for your child’s health-care providers to know what medications your child is taking at home, so that they can make sure your child is getting the right medications while in the hospital. After you fill out the Medication History Worksheet, show it to your doctor, nurse or pharmacist.
If you do not have access to the Internet, you can get a copy of a Medication History Worksheet at your SickKids clinic
