How to make a referral
Please click here for general information on referring patients to The Hospital for Sick Children.
Any medical doctor or practicing audiologist can refer patients.
New patients must be less than 18 years of age.
Information regarding a specific referral can be obtained by calling 416-813-6770 and referrals can be faxed to 416-813-6487.
Referring professionals and/or families will receive a response (appointment or redirection) within five (5) business days for non-urgent cases.
How to send your referral:
Send your referral via ARMS, our on-line Ambulatory Referral Management System or
Fax a completed referral form (available in PDF format) View the clinical directory for fax numbers.
