Can my Personal Health Information be sent via email or over the phone?

To protect your privacy, we do not release Personal Health Information via email or over the phone.

How do I request a CD for diagnostic imaging services?

The Health Records Department provides diagnostic imaging reports only. If you require diagnostic images (MRI, CT scans, Ultrasound) on CD, please fax a completed Consent for Disclosure of Personal Health Information form to the Diagnostic Imaging Department 416-813-6043. The Diagnostic Imaging Department can be contacted at 416-813-7511.

Do you automatically send a copy of the discharge summary to my physician following discharge from the Emergency Department or Inpatient unit?

No. Upon discharge, a copy of the discharge summary is given to the patient/parent to provide to the family physician/paediatrician. Your physician can also access this information via eCHN (Electronic Child Health Network) if they practice in Ontario or submit a written request to the Health Records Department.

Do you provide my physician a copy of the clinic note after my outpatient visit?

Yes. Clinic notes are sent to the referring physician/family physician when contact information is on file (check at time of registration). Your physician can also access this information via eCHN if they practice in Ontario or submit a written request to the Health Records Department.

Do you automatically send copies of test results to my physician?

No. Your physician can access this information via eCHN if they practice in Ontario or submit a written request to the Health Records Department.

Please Note:  Outside-ordered labs or diagnostic imaging results are sent directly to the ordering physician. These results can only be obtained from the ordering physician and not the Health Records Department.

How do I submit a request for legal or insurance purposes?

Please submit a written request to the Health Records Department.  An original consent is required before your Health Record can be released to lawyers or Insurance companies. There is a fee associated with this service.

When can I expect my request to be completed?

In accordance with the Personal Health Information Protection Act (PHIPA), the Health Records Department will process your request within 30 days.  If your request is urgent please specify on the consent form.

Is there a fee for requesting copies of health records?

Fees apply. Please refer to the section on administrative fees for more information.

Can I request access to my child’s health record without their consent?

Legal authorization is required to access your child’s Health Record. Please submit a written request to the Health Records Department.

How do I obtain records for an incapable patient?

If the child is not capable of signing the consent, the legal guardian or a substitute decision maker must submit a written request to the Health Records Department.  Proof of incapability or guardianship status may be required.

Can I review my health records?

Patients may review their health record. Please submit a written request to the Health Records Department. An ROI specialist will contact you to set up an appointment. Please note that fees apply for this service. Please refer to the administrative fees section.

How can I request correction to information contained in my record?

Submit a request in writing to the Health Records Department. Please refer to the section on Correction to your health record for details.

Where do I go to submit a request for access to my health records?

Please visit The Hospital for Sick Children’s Health Records Department in room S203. You may also submit your request by mail or fax. Please refer to the written request section for more information.

Can I withdraw my consent for disclosure of my personal information?

Patients may withdraw their consent at any time. Please notify the Health Records Department in writing if you no longer consent to the disclosure of your Personal Health Information.

What happens if I am inquiring about records of a deceased patient?

Please submit a written request to the Health Records Department. Proof of your legal signing authority is required.

Will the hospital have all of my records on file?

Inquire whether or not SickKids has your health record, please submit a written request to the Health Records Department.
