Resources for Youth and Adults 

For many patients with chronic health conditions and/or disabilities, a major aspect of transition planning is learning about and connecting to the appropriate supports for adults. While your health-care providers are working to support your “medical” transitions, these handouts describe a number of other transitions that will happen in your life, according to the following categories.
