Compounding service
Here at The Hospital for Sick Children (SickKids) we often encounter problems with the practicalities of administering the appropriate doses of certain drugs to paediatric patients. The obvious answer is to use a liquid formulation so that small doses can be measured. As a liquid it is also easier to administer. Our first consideration, however, is whether the dose can be accurately measured by halving or quartering a tablet. In this way there is no concern about stability, except if the tablet is a long-acting product.
The part tablet can be crushed and mixed with ice cream, chocolate sauce or applesauce and given to the child. This is often easier and cheaper than making a suspension. Powder packets could be made by crushing the tablet(s) and triturating with lactose to give the correct dose in each packet. The powder can then be given in the same way as above. Below are the SickKids liquid formulations and instructions to make the ones most commonly requested from us.
We recommend that our formulations and instructions are followed strictly since any changes may affect the stability of the product. In some of the formulations we have specified a particular brand of medication, again, changing the brand may affect the stability of the suspension. If the listed flavouring agent is not available then the formulation can be made without the flavouring without affecting the stability in any way.
