The Ambulatory Referral Management system (ARMs) is now being used in a majority of the clinics. Please browse through our directory to see the contact information for the clinics. Because not all clinics listed in the directory are online, they may be linked to their program’s website to provide you with additional referral information.

The contact list for our ambulatory clinics appears below. SickKids now uses four- and five-digit telephone extensions. If you are trying to reach a four-digit extension, you must dial 416-813-7654 press 1 and then the extension or press 2 and then the extension for a five-digit number.

Please use the ARMs fax number for sending in new referrals or supporting referral information only. For all other correspondence please use the clinic’s general fax number. If no ARMs fax number is listed, the clinic is not currently online, so use the general fax number or call the clinic for more information.
