Cardiology Referral Criteria
Who can refer
All community providers, Nurse Practitioners
Patient age
18 years of age and under
Patient acceptance
Suspected or known cardiac condition
- Transfer of care.
- Require supporting clinical documentation.
- Complete referral required
Conditions to be referred to other clinics/care teams:
- Postural Orthostatic Tachycardia Syndrome (POTS), refer to pediatrician or a community cardiology
- Hypertension with no arm leg gradient
Rejection criteria
- Asymptomatic murmur > 2 yrs of age, refer to community cardiologist
- Chest pain with no abnormal cardiac findings, refer to community cardiologist
- Faxed referrals are not accepted
Required supporting documentation
Check all the documentation needed for the specific conditions listed below:
- Arrhythmia/Palpitations: Consult note.
- Cardiac desynchrony: Consult note, detailed medical history, cardiac investigations completed, cardiac medications.
- Cardiomyopathy and Heart function assessment: Consult note, Vital signs, ECG and echo if completed.
- Chest pain: Consult note, any cardiac investigations completed (all) (ecg, echo), vital signs.
- Hyperlipidemia: Consult note, fasting lipid profile, growth chart.
- Inheritable arrhythmia disorders / Sudden cardiac death: Consult note, detailed medical history, ECG, all cardiac investigations completed (with tracings), cardiac consult note of affected family member including all cardiac investigations, genetic test results, autopsy report if applicable.
- Kawasaki disease: Consult note, discharge summary, ECG, ECHO.
- Known congenital heart defect: Consult note, Vital signs, ECG and ECHO if completed.
- Marfan syndrome/Connective tissue disorders: Detailed medical history, cardiac investigations completed, cardiac genetics report.
- Murmur: Growth chart (Height/weight, percentiles), Vital signs, Detailed medical history, Detailed assessment for symptoms of heart failure.
- Other: Medical history, cardiac investigations completed.
- Pulmonary hypertension: Vital signs, Detailed medical history, including all test results and oximetry level if available, Detailed assessment for symptoms of heart failure.
- Syncope/Pre Syncope: Medical history, cardiac investigations completed.
- Syndrome with associated cardiac anomaly: Medical history, cardiac investigations completed, Physical activity assessment, exercise med referral, exercise concern, medical history, echo, cardio test, lipid profile.
