Clinical & Metabolic Genetics Referral Criteria
Who can refer
Physicians, Nurse Practitioners
Patient age
Under 18 years of age.
Patient acceptance
Referrals to Clinical or Metabolic Genetics include patients requiring further medical assessment, testing and/or management recommendations, residing within the SickKids catchment area and/or have coordinated multi-specialty care at SickKids. Otherwise, please refer patients to their closest genetic centre. Referrals for genetic counselling include positive family history and requests for cascade testing.
Notes
If the mother is pregnant, please refer both mother and child to the Prenatal & Medical Genetics Clinic at Mount Sinai Hospital.
Required supporting documentation
For patients that have developmental delay/intellectual disability please include results of chromosomal microarray and Fragile X testing and other relevant information.
Include:
- Reason for referral.
- Specify urgency.
- All supporting medical information including previous genetic testing results (even if performed at SickKids).
- Any diagnostic imaging results.
- Relevant growth charts.
- Family history information.
