Diabetes Referral Criteria

Who can refer

Physician or Nurse Practitioner

Patient acceptance

Patients who have an established diabetes diagnosis and are on a management plan​. Please list any other comorbidities in your referral.

 

Rejection criteria

The Diabetes Clinic will not accept new diabetes diagnosis or patients who do not meet diagnostic criteria as per Diabetes Canada Guidelines. If a patient is not followed at a closer to home PDEP, the referral will be rejected and asked to be directed to community PDEP.  If the patient is already followed at a PDEP close to home, but consultation is required by SickKids Diabetes Team, then PDEP who follows patient will be asked to make the referral.

 

Notes

If you are referring for a new diabetes diagnosis, please direct the patient and family immediately to the nearest emergency department for assessment and initiation of treatment.  There are several Pediatric Diabetes Education Programsproviding excellent diabetes related care across the province and close to home; we encourage you to refer your patient to these programs first.

Required supporting documentation

​​Any bloodwork and/or complications screening from the last 6 months. Clinic letters from the last 6 months.
