Otolaryngology Referral Criteria

Who can refer

Physicians, Nurse Practitioner.

Patient age

Must be under the age of 17 for any referral.
Under age 10 for septoplasty/nasal obstruction.
Under age 12 for perforated tympanic membrane/Tympanoplasty.

Rejection criteria

We do not accept referrals for:

Hearing assessments

We do not accept referrals if children are otherwise healthy for the following conditions:

Otitis media with effusion/consideration of ear tubes
Obstructive sleep apnea or enlarged tonsils
OSA in child with Down Syndrome unless patient has a cardiac defect
Removal of tonsils for treatment of periodic fever, preauricular sinus, tympanoplasty in children 12 years and older,
Septoplasty/nasal obstruction in children 10 years and older
Second opinion unless requested by Otolaryngologist with specific need documented
Children who do not have any secondary health issues unless being referred for a surgical issue that is not managed in the community (I.e. Cholesteatoma, Thyroglossal duct cyst, Dermoid cyst etc),

Referral should be submitted by the Otolaryngologist, or accompanied by documentation from the Otolaryngologist. We do not provide second opinions.

If the child is being referred for cosmetic concerns, please refer to the Plastic Surgery clinic.

We do not accept referrals for investigation of genetic causes of hearing loss. Genetic testing and imaging can be ordered directly by the community physician.

Notes

Children with microtia should be referred if the family is interested in hearing rehabilitation.
If the referral is for a hearing assessment, please refer to SickKids Audiology.

Required supporting documentation

All referrals must include a detailed reason for referral. If referring for hearing concerns, the referral must be accompanied by an audiogram (can be done at a community hearing center). If imaging for any referral has been done it must accompany the referral. If patient has seen a community Otolaryngologist/ ENT, consult notes must accompany the referral. If community Otolaryngologist has performed surgery, operative notes, supporting imaging must accompany the referral. The referral will not be triaged until these documents are received.
