Background and clinical significance

ANCA-associated vasculitis is defined by autoantibodies to proteinase 3 (PR3-ANCA) and myeloperoxidase (MPO-ANCA), which form the main serologic subtypes. PR3-ANCA is typically associated with a cytoplasmic (C-ANCA) pattern and is most often associated with granulomatosis with polyangiitis, while MPO-ANCA shows a perinuclear (P-ANCA) pattern and is more commonly associated with microscopic polyangiitis, eosinophilic granulomatosis with polyangiitis, and renal-limited disease. A positive result is not diagnostic on its own and must be interpreted in the clinical context of the patient.
