Alternative Format Request Form

Written information and other forms of communication are available in an accessible format upon request. Alternative formats such as large print, braille, plain language, alternative languages, or accessible electronic formats are available at SickKids as part of our ongoing Accessibility commitment.

To request information in an alternative format, tell your health-care team or clinic what you need and how they can support you, so an appropriate alternative format can be provided. If you need further assistance, submit a request by filling out the following request form, emailing accessibility.questions@sickkids.ca, or calling 416-813-7654 ext. 228315.

Temporary Service Disruption Notice: Ext 228315 currently goes directly to voicemail as we change to a new system. The voicemail is monitored and you will receive a call back in 3-5 business days. If you have an urgent request or feedback, please contact Patient and Family Relations.
